A Board-certified Pediatrician, Dr. Cannon is a senior healthcare executive with 20+ years of progressive experience in clinical and administrative environments and a demonstrable record of improving the overall value of healthcare delivery.
Dr. Cannon founded DMC Healthcare, PLLC consulting firm in 2013. Client impact includes improving patient satisfaction, increasing size of overall patient panel, patient retention/through-put, and overall clinic efficiency/productivity for a Texas Child Protective Services (CPS) Medical/ Dental Clinic. For a Texas-based managed care organization, he improved utilization management, cost containment and medical quality. He currently provides advisory and clinical leadership to a start-up healthcare technology company that leverages AI and ML to improve patients' health outcomes and reduce the total cost of care. DMC Healthcare, PLLC also provides ongoing advisory and clinical support to a not-for-profit managed care organization in the areas of utilization & case management, cost containment, and quality improvement functions.

As Vice President, Chief Outcomes Officer, Joint Pediatric Enterprise (JPE) at Children’s Health System of Texas (2021), Dr. Cannon collaborated with internal/external stakeholders to work on executing on JPE’s mission to improve quality, cost/efficiency of care for the growing North Texas pediatric population. Partnering with a technology company, he targeted reductions in length of stay, readmission rates and asthma-related admissions by operationalizing efforts to prevent and decrease sepsis incidents.
Before, as VP & Medical Director, Children’s Medical Center Health Plan (2018 –2021), and as Medical Director, Children’s Medical Center, Asthma Management Program, Dr. Cannon oversaw/managed the health plan's clinical operations teams and developed results-driven and cost-effective solutions. Working with HMS and Quality Management Directors, he ensured meeting/exceeding healthcare cost and quality targets by developing/implementing processes to manage clinical policies that ultimately led to improved Plan profitability, achieving compliance for the first time in Firm’s history, improved medical/behavioral health outcomes (40% reduction in potentially-preventable readmission rates) and improved quality of care.
From 2015 – 2018, Dr. Cannon served as Medical Director for Amerigroup (Anthem, Inc.), ensuring that the appropriate medical care was delivered in the most cost-effective manner and in the most appropriate setting.
Dr. Cannon started his career as an Attending Physician with Wolfson Children’s Hospital, Emergency Room/Urgent Care Center in 2001 and held both Attending Physician and progressive Medical Director roles with several organizations, the last as Senior Medical Director, Community Health Choice (2014 –2015). From 2007 – 2008, he also served as Clinical Assistant Professor in addition to his role as Medical Director at the University of Florida/Kids ‘N Care Health Center, Jacksonville (2007 – 2008).
Dr. Cannon has an MD from the Louisiana State University School of Medicine, an MS in Healthcare Leadership & Management from the Naveen Jindal School of Management, and a BS in Biology cum laude from Xavier University of Louisiana. He completed his Residency in General Pediatrics at the University of Florida in Jacksonville. Dr. Cannon is board certified in General Pediatrics and is a Fellow of the American Academy of Pediatrics. He is also a Fellow, Texas Hospital Association Leadership Fellows Program (2021).
Dr. Cannon enjoys spending time and traveling with his wife and their two beautiful daughters. He and his wife regularly participates in competitive races of various distances across the country and abroad.

A patient-focused clinical integration executive, Amy has had progressive responsibility in leadership positions serving the Health Care industry. She was part of team that built a health plan from the ground up, provided care for people with complex needs and ensured cost and process optimization. Her expertise includes people management and leadership, quality care, value-based models of care, program planning, implementation and evaluation, case management and service coordination. Her deep knowledge of policies governing healthcare delivery and planning and her collaborative spirit inform all her work.
During her last role as VP, Clinical Integration at Childrens’ Health Systems of Texas (CHST) from 2015 to 2020, Amy was responsible for overall performance and accountability of five Population Health programs that included product performance and contract negotiation as value-based care initiatives within the Accountable Care Organization (ACO). Managing multiple teams and programs, Amy expanded programs and decreased costs. She was part of the Executive Team that coordinated and improved patient care, reduced inpatient utilization, and enabled passing operational/state compliance readiness review by successfully building and launching a new Medicaid Managed Care Organization and subsequently creating the Children’s Medical Center Health Plan in 2016 for children with complex medical issues (STARkids). She also enhanced CHST’s footprint (+9000 members) and community recognition, and was involved in all presentations and high-profile meetings with HHSC.
Amy joined CHST as Director of Health Management Services (2014 – 2015), and started to build the infrastructure for the new Plan at that time.
Earlier, Amy served as Health Services Manager and then as Health Services Director at United Healthcare Community and State of Texas (2011 – 2015). As Manager, she improved patient care and decreased utilization of patient admission by implementing Hospital to Home program for high-risk Members; played a critical part in driving $1M savings to the Health Plan the first year of the program launch; and improved quality metrics by co-leading with Health &Human Services (HHSC) the Wellness Incentive Navigation successful pilot implementation targeting better care through more intensive care management. As Health Services Director, managing and developing a team of 58 registered nurses, and serving as part of Provider’s outreach team, Amy enabled an additional 4,000 people with complex medical needs in the Northeast Service Delivery Area to choose to live and receive care in a home setting instead of a nursing facility; increased capacity by growing the number of partnerships with health care providers; and enabled the Organization to pass service coordination readiness reviews. In 2010, Amy received the United Health Care Community and State Sage Award for clinical services.
Amy started her career as a Registered Nurse, Labor/Delivery at Parkland Memorial Hospital, Dallas/Ft. Worth Area in 1987 and served several Health Care organizations in TX and also in MA (Massachusetts General Hospital).
She received an MBA in Healthcare Management from Western Governors University in Salt Lake City, UT, a Bachelor of Science in Nursing from Baylor University, Waco, TX, and received her Case Management Certification from the Commission for Case Managers in 2011.
Amy enjoys spending time and traveling with her husband, three daughters and their families. She and her husband are avid sports fans and enjoy attending musical performances.
Jason Buchanan, MD, MS, FAAFP

Dr. Buchanan brings to the table over 15 years of experience in the health care industry. He is a clinical informaticist and a practicing family physician. He holds a master’s degree in Biomedical Informatics, with a specialization in Clinical Informatics, from The University of Texas at Houston Health Science Center School of Biomedical Informatics. He is broadly trained in all aspects of informatics and at present his work focuses on data and predictive analytics, telemedicine, electronic health record optimization, workflow construction and redesign, and health information technology training and education. His tenet is to use data and health information technology to improve health outcomes, maximize revenue, minimize expenditures, and improve the overall quality of life for patients and providers.
From modest beginnings, Dr. Buchanan was born and raised in a tough section of New York City. Guided by a wonderful family and supportive teachers, he made his way to New Haven, Connecticut where he attended Yale University for his undergraduate studies. At the age of 20, he graduated from Yale with a Bachelor of Science degree in Psychobiology (Intensive Research track). He was then recruited to Houston, Texas for medical school and obtained his medical degree from the Baylor College of Medicine. He subsequently completed his residency training in Family Medicine through the Houston Methodist Hospital System. He has remained in Houston as an assistant professor of Family and Community Medicine where he has provided outstanding direct patient care to thousands of patients and has won numerous recognitions and awards for his service and quality of care.
Dr. Buchanan is board certified in Family Medicine and is a Fellow of the American Academy of Family Practice. Professional memberships include the American Medical Informatics Association (AMIA), American Association of Physician Leadership (AAPL), and the American Association of Family Practice (AAFP). He delights in being a husband and adores being a father to two children. Dr. Buchanan is an avid runner and enjoys weight training, fishing, and taking time to appreciate the things often overlooked in life.
